Breast Implant (Breast Augmentation)

Implant or fat transfer procedure to enhance breast size and shape

Process Overview

Patient’s Experience

What Is Breast Implant?

The term Breast Implant refers to surgical techniques that enhance the size, volume, and shape of the breasts. It is commonly performed using either silicone implants or fat transfer, depending on the patient’s anatomy and desired look. As in all surgeries, patient safety is prioritized by following strict protocols. Before surgery, a comprehensive evaluation of the patient’s health history and current condition is performed. If the patient is found to be a suitable candidate, preoperative testing is conducted. Once cleared, the surgery is performed with full preventive measures in place against complications such as infection, bleeding, and thromboembolism.

Throughout the operation, patient vitals are continuously monitored using advanced systems. Careful attention is also given to postoperative monitoring to detect any early warning signs of complications. Antibiotic prophylaxis is administered to reduce infection risk. Anti-bleeding medications are used, and surgical techniques are refined to minimize blood loss. To prevent deep vein thrombosis, the patient is positioned properly, wears pneumatic compression stockings, and receives heparin if indicated. In implant procedures, sterile techniques and anatomical positioning are strictly followed. For fat transfer cases, only fat harvested from appropriate areas is processed and injected into the breast tissue without entering the muscle. Ultrasound guidance may be used to ensure safe placement. All these steps ensure the safest surgical conditions.

How Can I Use My Own Body To Enhance Breast Shape?

In patients with regional fat accumulation—such as in the abdomen, flanks, thighs, or arms—and small or deflated breasts, fat transfer breast implant offers a dual benefit. Fat can be harvested from excess areas and used to enhance the shape and volume of the breasts. This method is especially ideal for those seeking a more natural look or wishing to avoid implants. The fat is extracted through liposuction, processed using special techniques, and injected into areas of the breast that need more volume—especially the upper pole or cleavage line.

This technique not only improves breast aesthetics but also sculpts other areas of the body, enhancing the overall silhouette. The fat cells are carefully filtered to maximize their survival after transfer. Our goal is to achieve full, natural-looking breasts with balanced contours and minimal scarring. For patients with poor skin elasticity, additional skin-tightening treatments like J-Plasma may be included. Whether through fat or implant, each case is custom-planned according to body proportions, expectations, and safety criteria to ensure a satisfying and natural result.

I had my surgery with Dr. Gonullu back in December 2021. I am almost a full month after my tummy tuck, 360 Lipo, and a breast lift. I am honestly very happy with the results and this has been such a huge self-esteem booster. Dr. Gonullu was very helpful, informative, and realistic about what to expect. He did a very beautiful job.

“I was most impressed with Dr Ersin’s kindness, professionalism, and expertise. He took the time to really explain the lifting procedure. I originally thought I wanted implants as well, but he explained why he didn’t think it was necessary and I appreciate him looking out for my best interest. I’m 5 weeks post op and absolutely thrilled with my results! I know they will get even better with time, but they look better than I imagined was possible. Dr Ersin and his staff were wonderful, extremely helpful and responsive throughout the entire process. Highly recommend!!”

Who Is A Good Candidate For Breast Implant?

Breast implant is ideal for individuals who are physically healthy, maintain a stable weight, and are looking to enhance the size, shape, or symmetry of their breasts. Common reasons include naturally small breasts, loss of volume after pregnancy or breastfeeding, asymmetry between the breasts, or changes in breast shape due to weight fluctuations or aging.

Candidates should also have realistic expectations and a clear understanding of the limitations and possibilities of the procedure. Breast implant can improve self-esteem and body image, but it is important to note that perfection is not the goal — rather, natural enhancement and balance with the rest of the body. Good skin quality and sufficient breast tissue are also factors that contribute to better outcomes.

Types Of Breast Implants And Personalized Choices

Breast implant is not a one-size-fits-all procedure. Today, patients can choose between saline and silicone implants, each with unique benefits. Silicone implants are the most popular due to their natural feel and realistic look, while saline implants offer adjustability and are inserted empty, then filled during surgery, allowing smaller incisions.

Additionally, implants come in a variety of shapes (round or anatomical/teardrop), sizes, and projections. Round implants offer more upper pole fullness, while anatomical implants create a more natural slope. Textured vs. smooth implant surfaces and cohesive gel (“gummy bear”) options are also discussed during consultations. The final choice depends on the patient’s chest anatomy, lifestyle, and aesthetic goals — with detailed guidance provided by the surgeon.

Implant Placement And Surgical Techniques

Implants can be placed either subglandularly (above the chest muscle but under the breast tissue) or submuscularly (beneath the pectoral muscle). Submuscular placement generally gives a more natural result and reduces the risk of visible implant edges or rippling, especially in thinner patients. However, subglandular placement may offer a faster recovery and is sometimes preferred for those with more natural breast tissue.

Incision options include inframammary (in the breast fold), periareolar (around the nipple), and transaxillary (in the armpit). Each has specific advantages in terms of scar visibility, access, and healing. During the preoperative planning process, the surgeon will help determine the best surgical plan based on the patient’s body structure and preferences.

Safety And Advanced Surgical Protocols

Breast implant is a safe procedure when performed by a skilled surgeon in an accredited facility. We take several steps to ensure safety, including thorough preoperative evaluations, lab testing, breast imaging if necessary, and anesthesia consultations. Surgical risks such as bleeding, infection, capsular contracture, or implant displacement are minimized through meticulous technique and post-op monitoring.

We use advanced surgical techniques and precise dissection methods to minimize trauma to the surrounding tissue. Antibiotic prophylaxis, sterile handling of implants, and careful pocket creation further reduce complication risks. Patients are closely monitored during and after surgery to ensure stable vitals and immediate response to any potential concerns.

Pain And Recovery Process In Breast Augmentation Surgery

Recovery after breast augmentation is generally well-tolerated. Patients are usually discharged on the same day as the surgery. Pain is manageable with prescribed medications, and many return to light daily activities within a few days. Most patients resume work within 5–7 days, depending on the nature of their job.

Swelling, tightness, and slight bruising are common in the early stages. Supportive surgical bras are worn for several weeks to aid healing, protect the implant position, and reduce discomfort. Physical activity, especially involving the upper body, is restricted for 4–6 weeks to avoid implant shifting. Follow-up appointments are scheduled to ensure proper healing and address any questions during recovery.

How Natural Will My Breasts Look And Feel?

One of the most common concerns is whether augmented breasts will look and feel natural. With today’s advanced implant options and placement techniques, the results can be extremely lifelike. Silicone implants, in particular, offer a texture and movement that closely resembles natural breast tissue. Implant shape, size, and placement are all carefully chosen to match the patient’s body.

Patients with ample natural tissue may see more immediate natural-looking results, while slimmer individuals may take a few weeks for swelling to settle and implants to “drop and fluff.” Over time, the implants settle into position, and the breast tissue adjusts, revealing a softer, more natural shape.

Long-term Care, Implant Lifespan, And Maintenance

Breast implants are not lifetime devices. While many patients enjoy their implants for 10–20 years without issue, implants may eventually require replacement due to natural aging, changes in body shape, or complications such as rupture or capsular contracture.

Regular checkups are important. We recommend annual breast exams and periodic imaging (MRI or ultrasound) to monitor implant integrity, especially for silicone implants. Patients should also be aware of long-term symptoms of implant rupture or rare complications such as Breast Implant Illness (BII) or BIA-ALCL — though these are extremely rare, especially with smooth implants.

Breast Implant With Fat Transfer

For patients seeking a more natural or subtler enhancement, fat transfer breast implant is an option. This involves liposuctioning fat from areas like the abdomen or thighs, purifying it, and injecting it into the breasts. This method avoids implants altogether and offers the added benefit of body contouring in the donor areas.

While fat grafting cannot achieve the same volume increase as implants, it’s a suitable choice for those wanting 1–2 cup sizes of enhancement with no foreign materials. Fat survival varies but typically ranges between 50–70%. In some cases, a second fat transfer session may be needed for optimal results.

Emotional And Aesthetic Benefits

Breast implant often leads to improved confidence, body positivity, and satisfaction with one’s appearance. Patients frequently report a renewed ability to wear clothing they previously avoided, such as swimwear or fitted tops. Many also feel a significant boost in femininity, posture, and overall self-expression.

While the physical change is the most visible, the psychological impact can be equally meaningful. We work closely with patients to set realistic expectations and ensure that they’re making the decision for the right reasons — ultimately leading to high satisfaction and lasting results.

How Long Do I Need To Be Careful With My Upper Body Movements?

During breast augmentation, especially with implants or fat transfer to the upper breast pole, special care is needed to avoid disturbing the surgical site. For approximately 6–8 weeks, patients should avoid lifting heavy objects, raising their arms above shoulder height, or applying pressure to the chest. This helps protect the newly positioned implants or fat grafts. It’s also important to sleep on your back, slightly elevated, to avoid pressure on the breasts. Wearing a post-surgical bra continuously during this time is crucial. These steps help ensure the long-term stability of the results and reduce the chance of complications like implant displacement or fat loss.
